" a 2-millimeter hole was discovered in the hull of a Russian Soyuz capsule (Soyuz MS-09) docked at the International Space Station in August 2018.The incident unfolded as follows:Discovery: Ground control noticed a slight drop in cabin air pressure and traced it to the Russian section of the station.Repair: Astronauts successfully located the 2-millimeter puncture and plugged it using a specialized epoxy and gauze repair kit, permanently stopping the leak.The Cause: Officials from the Russian space agency (Roscosmos) stated the hole was caused by a drill, noting nearby scrape marks suggested it was done from the inside by a "faltering" or "wavering hand"."   

They don't know if it was done on the ground or not. 2mm is pretty small...but when you're far from home...
